Medical Form:

All non-Westminster students enrolling in enrichment classes or full-day camp must provide a current School Entrance Health Form.

Proof of Identity and Age:

All non-Westminster students enrolling in enrichment classes or full-day camp must provide proof of identity and age. Prior to the start of camp, parents must present an original birth certificate or passport to an office staff member at Westminster School. The office personnel will log the proof of identification and make a copy for the student’s file.

Emergency Contact Information:

Upon registration, parents will receive an emergency contact card. This card must be completed and returned to Westminster School prior to the start of camp. Westminster students are not required to provide this information as it is already on file at the school.
