Medical Form:

All non-Westminster students enrolling in enrichment classes or full-day camp must provide a current copy of a School Entrance Health Form. Additionally, parents must submit one completed Emergency Contact and Health Information Form for every child registered.

Proof of Identity and Age:

All non-Westminster students enrolling in enrichment classes or full-day camp must provide proof of identity and age. Prior to the start of camp, parents must present an original birth certificate or passport to an office staff member at Westminster School. The office personnel will log the proof of identification and make a copy for the student’s file.
